Graduation Certificate Program

As we move toward the end of the school year, Your Senator would like to honor your graduating and/or moving up student class with a Certificate of Achievement on this auspicious occasion. This program acknowledges your students' hard work and dedication by presenting them with a Senate certificate celebrating their accomplishment.
To participate in this program, please provide a list of potential graduates in an Excel file at least 1 week before your graduation date.
Requirements for the Student Submission Excel File:
1. The File should be saved as 2025GradCert.xlsx
2. Column A: Please provide a list with the student’s first and last name, listed as they should appear on the certificates. The names of your students should be capitalized and spaced appropriately for ease of production. i.e. Jane Doe or John D. Doe
3. Column B: Grade moving up or Graduating from i.e. 3K, Pre-K, Kindergarten, Middle School, or Highschool
4. Column C: School Name- How you would like it to appear on the certificate if applicable i.e. P.S. 182 or Public School 182 or Yonkers High School
5. Column D: Certificate Requested- Moving up or Graduating Certificate i.e. Moving up or Graduation
6. Column E: Point of Contact - First and Last name, phone number and email i.e. Jane Doe, 518-555-5555, janedoe@school.edu
7. Column F: School Address for certificates to be delivered to. FYI: We cannot send certificates to a P.O. Box Attn: Jane Doe 123 School Drive New York, NY 12345
