Requires that persons maintaining a license to receive, board or keep a child or minor, notify the local social services department of any change in persons residing at such home; failure of such license holder to notify the department shall be cause for revocation of such license; requires establishment of a toll-free hotline for reporting of such changes.

AN ACT to amend the social services law, in relation  to  providing  for
  mandatory  disclosure  of  persons providing care of children, and for
  the revocation of license for failure to give such notice

  THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND  ASSEM-
BLY, DO ENACT AS FOLLOWS:

  Section 1. Subdivision 1 of section 378 of the social services law, as
amended  by  chapter  555  of  the  laws  of 1978, is amended to read as
follows:
  1. Certificates or licenses to receive, board or keep any child and/or
minor shall be in the form prescribed and provided by  the  [department]
OFFICE OF CHILDREN AND FAMILY SERVICES to the effect that such person is
regarded  by  the  issuing authorized agency or [social services depart-
ment] THE OFFICE OF CHILDREN AND FAMILY SERVICES, as the case may be, as
maintaining a home suitable for the care of children and/or  minors  and
specifying  the name, address, and religious faith of the person to whom
issued, the number of children and/or minors for  whom  such  person  is
certified  or  licensed  to  care,  NAMES OF ALL ADULT AND MINOR PERSONS
RESIDING AT SUCH HOME AND THE NAMES OF ANY ADULT  PERSONS  WHO  PROVIDES
CARE, BABY-SITTING OR SUPERVISION OF THE CHILDREN RESIDING AT SUCH HOME,
and  such  other  information as the [department] OFFICE OF CHILDREN AND
FAMILY SERVICES may require. NOTWITHSTANDING ANY OTHER PROVISION OF LAW,
ANY CHANGE IN THE PERSONS RESIDING IN SUCH HOME, OR  PROVIDING  CARE  TO
ANY  CHILD SUBJECT TO THIS ARTICLE, SHALL BE IMMEDIATELY REPORTED TO THE
LOCAL SOCIAL SERVICES DEPARTMENT. FAILURE  TO  MAKE  SUCH  REPORT  SHALL
CONSTITUTE CAUSE FOR REVOCATION OF CERTIFICATES AND LICENSES PURSUANT TO
SECTION  THREE  HUNDRED SEVENTY-NINE OF THIS TITLE.  THE OFFICE OF CHIL-
DREN AND FAMILY SERVICES SHALL ESTABLISH AND CAUSE TO  BE  PUBLICIZED  A
