BILL NUMBER:S6460 TITLE OF BILL: An act to amend the public health law, in relation to requiring health care providers and practitioners to provide contact information for the NY Connects: Choices for Long Term Care program PURPOSE OR GENERAL IDEA OF BILL: This bill would require health care providers and practitioners to provide either a patient or their repre- sentative contact information for the NY Connects: Choices for Long Term Care program when there is a determination that the patient is in need of long term care services. SUMMARY OF SPECIFIC PROVISIONS: Section 1 adds a new section, 2997-e, to the public health law require health care providers and practitioners to provide either a patient or their representative contact information for the NY Connects: Choices for Long Term Care program when there is a determination that the patient is in need of long term care services Section 2 sets the effective date JUSTIFICATION: Implemented in 2009, the NY Connects program provides locally accessible, consumer centered access points to seniors seeking information about long term care options. Seniors are provided with linkages to services and are able to make informed decisions about their

VALESKY -- read twice and ordered printed, and when printed to be committed to the Committee on Health AN ACT to amend the public health law, in relation to requiring health care providers and practitioners to provide contact information for the NY Connects: Choices for Long Term Care program THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM- BLY, DO ENACT AS FOLLOWS: Section 1. The public health law is amended by adding a new section 2997-e to read as follows: S 2997-E. PROVISION OF CONTACT INFORMATION RELATING TO LONG TERM CARE. WHENEVER A HEALTH CARE PROVIDER OR PRACTITIONER MAKES A RECOMMENDATION REGARDING THE NECESSITY OF LONG TERM CARE SERVICES OR A REFERRAL FOR THE RECEIPT OF LONG TERM CARE SERVICES TO A PATIENT, THE PATIENT OR PATIENT'S DESIGNATED REPRESENTATIVE SHALL BE PROVIDED BY THE HEALTH CARE PROVIDER OR PRACTITIONER THE CONTACT INFORMATION FOR NY CONNECTS: CHOIC- ES FOR LONG TERM CARE, ESTABLISHED PURSUANT TO SUBDIVISION EIGHT OF SECTION TWO HUNDRED THREE OF THE ELDER LAW, THAT CORRESPONDS TO THE PATIENT'S COUNTY OF RESIDENCE OR PROSPECTIVE COUNTY OF RESIDENCE BASED ON THE PREFERENCE OF THE PATIENT.
