 AN ACT to amend the judiciary law, in relation to the form of the  juror
   qualification questionnaire
 
   THE  PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM-
 BLY, DO ENACT AS FOLLOWS:
 
   Section 1. Section 513 of the judiciary law, as added by chapter 86 of
 the laws of 1995, is amended to read as follows:
   § 513. Form of questionnaire. The questionnaire to be  filled  out  by
 prospective  jurors  shall be in such form as the chief administrator of
 the courts may prescribe; PROVIDED, HOWEVER,  SUCH  QUESTIONNAIRE  SHALL
 ONLY  REQUIRE  A  PROSPECTIVE  JUROR  WHO IS NOT A CITIZEN OF THE UNITED
 STATES TO CERTIFY, UNDER PENALTY OF PERJURY, THAT HE OR  SHE  IS  NOT  A
 CITIZEN  AND DOES NOT MEET THE QUALIFICATIONS UNDER SECTION FIVE HUNDRED
 TEN OF THIS ARTICLE.
   § 2. This act shall take effect on the one hundred eightieth day after
 it shall have become a law.
