LEGISLATIVE RESOLUTION honoring Dave Hoffman upon the occasion of his
designation as the 2015 Outstanding Community Advocate by the
Alzheimer's Association of Northeastern New York
WHEREAS, It is the sense of this Legislative Body to recognize that the
quality and character of life in the communities across New York State
are reflective of the concerned and dedicated efforts of those organiza-
tions and individuals who would devote themselves to the welfare of the
community and its citizenry; and
WHEREAS, It is incumbent upon the people of the State of New York to
recognize and acknowledge those within our midst who have made signif-
icant contributions to the quality of life therein; and
WHEREAS, This Legislative Body is justly proud to honor Dave Hoffman
upon the occasion of his designation as the 2015 Outstanding Community
Advocate by the Alzheimer's Association of Northeastern New York, to be
celebrated at its Gala Event, "An Evening to End Alzheimer's," on
Friday, May 1, 2015, at the Hall of Springs in Saratoga Springs, New
York; and
WHEREAS, The Alzheimer's Association is the world's leading voluntary
health organization in Alzheimer's care, support and research; and
WHEREAS, The mission of the Alzheimer's Association is to eliminate
Alzheimer's disease through the advancement of research, provide and
enhance care and support for all affected, and reduce the risk of demen-
tia through the promotion of brain health; and
WHEREAS, Dave Hoffman has been called upon to contribute his time and
talents to countless civic and charitable endeavors and has always given
of himself unstintingly; and
WHEREAS, Director of the Bureau of Community Integration and
Alzheimer's Disease in the Office of Health Insurance Programs at the
New York State Department of Health, Dave Hoffman manages all dementia
programming in the state, as well as several other initiatives; and
WHEREAS, This esteemed man is also a Clinical Associate Professor at
the State University at Albany School of Public Health and teaches
Medical Ethics at Maria College as an Adjunct Professor; in addition to
his other credentials, he also recently earned a post graduate certif-
icate in Bioethics at Albany Medical College; and
WHEREAS, Driven by the impact of seeing numerous loved ones lost to
Alzheimer's Disease, Dave Hoffman was instrumental in the passing of a
bill granting Alzheimer's Associations statewide the most significant
increase in funding in history; and
WHEREAS, A true asset to society, Dave Hoffman is a steadfast and
dedicated member of several advisory groups including the National Advi-
sory Council for Alzheimer's Disease Research, the National Campaign for
Public Health, the New York State Geriatric Mental Health Planning Coun-
cil, and the National Center for Chronic Disease Prevention and Health
Promotion at CDC; and
WHEREAS, Throughout the entire period of his community service, a
period of constructive involvement, Dave Hoffman has stood constant in
dignity, good grace and humor; and
WHEREAS, With him throughout have been his wife, Susan, and their two
children, Peter and Elizabeth, all of whom feel privileged to be a part
of his life and rejoice in his achievements; and
WHEREAS, Rare indeed is the impressive dedication shown by an individ-
ual for the benefit of others which Dave Hoffman has displayed through-
out his life; and
WHEREAS, It is the custom of this Legislative Body that when individ-
uals of such noble aims and accomplishments are brought to our atten-
tion, they should be celebrated and recognized by all the citizens of
this great Empire State; now, therefore, be it
RESOLVED, That this Legislative Body pause in its deliberations to
honor Dave Hoffman upon the occasion of his designation as the 2015
Outstanding Community Advocate by the Alzheimer's Association of North-
eastern New York, and to wish him continued success in all of his future
endeavors; and be it further
RESOLVED, That a copy of this Resolution, suitably engrossed, be tran-
smitted to Dave Hoffman.
