Statewide Health Care Facility Transformation Program Provides Opportunities to Health Care Providers

This year's State Budget included funding for the Statewide Health Care Facility Transformation Program. Thirty million dollars is available for community-based health care providers such as primary care providers, treatment and diagnostic centers, home care providers, alcohol and substance abuse treatment centers, and mental health clinics.

This program is intended to increase New Yorker’s accessibility to a financially sustainable system of health care. Upstate New York tends to be left out of major funding streams and this is one opportunity that the Southern Tier shouldn't pass up.

If your agency utilizes acute inpatient, outpatient, primary, home care of residential health care services within a community, this is a great chance for you to transform your facility.

Questions about the RFA must be sent to healthcarefacilitytransformation@health.ny.gov before August 10th.

Letters of interest are due by August 19th and applications must be submitted through the Grants Gateway by September 16, 2016.

The anticipated award date is December 1, 2016 and the estimated contract start date is March 1, 2017.
