In my ongoing effort to better serve you, I am always looking for new ways to improve how I can help constituents who need assistance. Issues can normally be resolved through a simple phone call or e-mail, but there are in fact times when a situation requires more involved communication. As a result, you may wish to share copies of correspondence and documents that will help me resolve the matter more quickly on your behalf.
To accommodate those who wish to include additional information, I would like to introduce you to my newest way to correspond - “Dear Joe, Please Help!”
By downloading this contact form and filling out all of the information requested, you may then attach additional documents and mail them directly to my office. This allows me to research your request more thoroughly and respond to you in a more timely manner. The contact form is made available both as a Microsoft Word file and as a PDF file for your convenience.
