Cease and Desist Questionnaire

Attached is the questionnaire handed out by the New York State Department of State at the Cease and Desist Public Hearings on 9/14 and 9/15. 

If any community members did not attend the hearing or receive a questionnaire, now is their chance to let their testimony be heard. Once the questionnaire is answered, community members are encouraged to bring the form and any other supporting evidence to Senator Avella's District Office.
