• Open to men age 40 and older who have not previously been diagnosed with
prostate cancer, regardless of whether or not they have insurance. (Screenings
are available for those who have not received a free screening from IMF in
the past 12 months).
• Screenings will involve a free PSA blood test.
• Men participating in the program must bring a photo ID listing their address
(such as a driver’slicense).
• Patients must bring a self-addressed,stamped envelope and health care provider’s
name and addressto be notified of the PSA blood test results.
• Appointments are necessary;space islimited.
